If you’re managing a chronic condition, Medicare can subsidise exercise physiology through a GP Chronic Disease Management (CDM) plan. At Revolution Personal Training (17A Market Street, South Melbourne), our ESSA-accredited exercise physiologists use evidence-based, individually prescribed exercise to help you manage conditions like type 2 diabetes, heart disease, arthritis and osteoporosis, improving your health markers, function and quality of life. We accept CDM referrals and work with your GP, all in a supportive gym with a 5.0-star Google rating from 126+ reviews.

Does Medicare cover exercise physiology?

Yes. Medicare subsidises exercise physiology when your GP refers you under a Chronic Disease Management (CDM) plan, also known as a GP Management Plan plus Team Care Arrangement. Under this scheme you can receive up to five Medicare-subsidised allied health sessions per calendar year, shared across all the allied health providers you see. Medicare pays a set rebate per session and you pay a small out-of-pocket gap. No specialist referral is needed; just ask your GP.

Who is eligible for a Medicare CDM plan?

CDM funding is for people with a chronic condition, generally one that has lasted, or is likely to last, six months or more. Common examples include type 2 diabetes, cardiovascular (heart) disease, high blood pressure, arthritis and other musculoskeletal conditions, osteoporosis, obesity and metabolic conditions, and chronic pain. Your GP decides whether a CDM plan is appropriate and whether you’d benefit from exercise physiology as part of it. If you have one of these conditions, it’s worth asking at your next appointment.

How do I get Medicare-funded exercise physiology?

Book a GP appointment and ask whether a Chronic Disease Management (CDM) plan that includes exercise physiology is right for you. If your GP agrees, they’ll prepare the plan and provide a referral to an Accredited Exercise Physiologist (that’s us). Bring the referral to your first session at Revolution Personal Training. We’ll assess you, design your program, and send progress updates back to your GP so your care stays coordinated.

What conditions does exercise physiology help manage?

Exercise physiology is one of the most effective, evidence-based ways to manage many chronic conditions. For type 2 diabetes it improves blood-sugar control; for heart disease and high blood pressure it improves cardiovascular fitness and health markers; for arthritis and joint pain it builds strength and reduces pain; for osteoporosis it uses targeted resistance and weight-bearing exercise to support bone health; and for obesity and metabolic conditions it supports sustainable weight and lifestyle change. Every program is prescribed to your specific condition and capacity.
